Giving Permission for Someone to Access Your Medical Info

Now that you’re over 16, you can choose to let someone else (like a parent or carer) talk to us about your health and see your medical info. This includes things like:

  • Test results
  • Clinic letters
  • Notes from your appointments
  • Other info on your medical record

To do this, you’ll need to come to the reception and fill out a consent form. You have to sign it in person – we can’t accept online forms because your real (wet) signature is needed.

Important:
If you don’t fill out a consent form, we can’t share your medical info with anyone, even if they call for you.

You can also change your mind later. If you want to remove consent, just contact us and we’ll update your records.


Online Services for Over 16s

You can now sign up for online health apps like the NHS App or Patient Access. These apps let you do things yourself, without having to call or visit us:

  • Check your test results
  • Order repeat prescriptions
  • Read clinic letters
  • See notes from your appointments
  • View your immunisation history
  • Look at your medical conditions
  • Check your booked appointments
  • See any recent referrals

Step 1) Register for an account with your chosen app

Step 2) Complete this online services registration form on our website: Online services registration form

Step 3) Once your registration form has been processed by the surgery you will receive an email with an account ID, ODS code and linkage key. These will need to be entered into your app to link to your GP records.

Ordering prescriptions

You can order your prescription using the NHS App or Patient Access, like we mentioned above.
