We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

Please note our registration turn around time is up to 5 working days.

Further Information

The surgery is situated in Oakmount Road, Chandlers Ford near the junction with Winchester Road (opposite Waitrose). The Practice shares a Car Park with the Chandlers Ford Library. The Car Park is situated at the rear of the Library and is accessed from Oakmount Road. There is also a public underground car park at the rear of the Waitrose car park. Unfortunately we have no room for patients to park in the very small private car park behind the surgery and residential flats.

When registering you will be allocated a named GP, however, you will still be able to see any GP within the practice team. 

Please note that your medical records may take several weeks to arrive at the Practice.

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
