The following information is intended for all new patients that have registered at the surgery. We believe the information on this page will be useful to you, as our processes may be different to your previous surgery. Online services:
We encourage use of NHS app or patient access to order your prescriptions and view test results. If you have an account set up already and would like to receive new linkage keys, follow the link here. If you have not previously been set up for a new account, follow the link here for further guidance.
Please note: your first prescription request will need to be completed either in person at reception, or via email to [email protected]. Once your first order has been complete, your medications will then show on your repeat list on the app.
Consent:
If you would like to give consent to a third party to access your medical records on your behalf, for example your partner or carer, you will need to complete a consent form. This can be downloaded here. Once complete, please either hand in to reception or email the complete forms to [email protected].
Parents/Guardians of under 5’s:
We require full immunisation records for those under 5. If you have not provided this information for your under 5 year old, please email a copy of their full immunisation records to [email protected]
Test results
If you have recently had tests completed at your previous surgery, but have not yet had your results, we may not be aware. Please contact reception if you are pending test results.
If you have tests completed by our clinical team, as standard we will only contact you with an update on your results if they are abnormal. If for example you have had blood tests completed, you will need to contact reception for your results 2-5 days following the test if you have not heard from our team. We allow view to test results on the NHS app/patient access, if you’d like to register for these click here for guidance.
